-Coordinates total client services by conducting comprehensive assessment, monitoring the client's condition, promoting sound preventive practices, coordinating services and teaching and training activities through regular assessment.
- Evaluates the effectiveness of nursing service to the client and family on an ongoing basis.
- Performs admission, transfers, re-certifications, resumption of care and discharge for the home care client.
- Create and update client service plan per policy.
- Prepares and presents client's record to the Clinical Record Review Committee.
- Consult with physician concerning client's service plans.
