Schedule is Monday, Wednesday, Friday 9 am to 2 pm Job Responsibilities: Diagnoses: Throat cancer metastasized to his esophagus and liver, Diabetes, Arthritis in his Back Allergies: None Known to food or medicine Goals and Objectives: Empty & rinse portable urinal as needed. Ask/remind/assist him regarding his meds. Record on sheet, the times and dosages of his pain meds ensuring they are not taken more often than as directed. Do 1 small load of his laundry each shift, fold and put away. Change bed linens when requested. Standby assist for drying off. Assist with dressing. Offer and encourage foods or drinks. May take to Dr appts or run errands for him in his vehicle. Engage positively and upbeat with Eric and Renea, staying busy (not on phone). Assist client to organize areas of home if requested. Take out trash at end of each shift ADL's: Mobility: Client uses a walker and sometimes walks without one, but he will need a transport chair for distances. He will need help getting into a car. Bathing/Dressing/Grooming: Standby assist with his shower, warming up water. Ask if he wants help with washing. Assist out of the shower and ask if help needed with drying. Always assist with dressing. Offer socks as his feet are often cold. He can pick out his own clothes. Eating: Client can feed himself but reports that he does not have any tastebuds and does not get hungry anymore. He needs to be encouraged to take in any calories he can tolerate. Offer and encourage foods or drinks, offering a straw with drinks. He does occasionally vomit after ingesting even water, so do so slowly in very small increments. He may need assistance holding drinks. He is supposed to have something on his stomach when taking pain meds. Toileting: Client can get to the bathroom on his walker by himself for bowel movements but often uses the portable urinal for urinating that needs to be emptied and cleaned after each use. He toilets independently, but CG should ask if he needs help (wiping). He does not wear depends but has occasional accidents. Communication: Client speaks in a low mumbled tone, that is occasionally difficult to hear or understand. He is coherent, but often sleepy as the meds make him tired. He has no hearing loss but CG needs to talk to him slowly using short sentences. His receptive ability appears to be slowed. Qualifications: Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member Ability to effectively communicate through written and verbal language Valid Driver's License and adequate auto insurance Consent to criminal, elder abuse, motor vehicle and sex offender background checks Consent to drug and alcohol testing TB test
