Licensed Practical Nurse (LPN) Job Description
Human Touch Home Care is hiring a Licensed Practical Vocational Nurses in the Colorado area. The Licensed Practical Vocational Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse (RN). The LPN is responsible for delivering high-quality care to patients by performing a variety of clinical duties such as plan of care and providing treatments. As well as administrative tasks, assisting with patient assessments, working collaboratively with the members of the team to help meet positive patient care outcomes, and ensuring the comfort and well-being of patients. Responsibilities Provides direct patient care as defined in the state Nurse Practice Act Implements plan of care initiated by the registered nurse Provides accurate and timely documentation consistent with the plan of care Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, SLP) to the registered nurse and/or Administrator/DON Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique Participates in personal and professional growth and development. Performs other duties as assigned by the registered nurse Participates in rotational clinical on-call (minimal) Participates in once weekly case conference Agrees to occasionally assist with CNA visits, in the absence of CNA The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job. Qualifications Graduate of an accredited practical nurse or vocational nursing program Two (2) years nursing experience. Community/home health or medical/surgical experience is preferred Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of Colorado or possesses a compact licensure Possess and maintain valid IV and CPR certification Complies with accepted professional standards and practice Demonstrates good verbal and written communication and organizational skills Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements Must have reliable transportation and company-required auto liability insurance. Willing to travel within 35 miles radius.
