Certified Nursing Assistant (CNA) Job Description
Clay Health Care Center is seeking an Certified Nursing Assistant (CNA) for our Ivydale, WV location. Clay Health Care Center joined the AMFM Family July 1, 2014 and is a 60 person care center specializing in skilled nursing, short-term rehabilitation to home, and long-term care services. Summary: We are seeking compassionate, hardworking, dedicated individuals to provide direct care to acutely and/or chronically ill patients/residents through individualized care plans, in compliance with state and federal standards, rules, and regulations, and facility policies and procedures, under the supervision of a licensed nurse. Responsibilities: Understand all rights offered to residents and act accordingly. Identify patient problems and concerns and report them immediately to a charge nurse or a licensed nurse. Complete documentation accurately and consistent with policies and procedures. Accurately input documentation into kiosk. Ensure patients are given adequate assistance with meals, nourishment, and transportation to meal service areas. Ensure patients are given adequate assistance with transportation to activities and special functions Ensure completion of assignments timely and accurately. Conduct patient rounds as assigned. Always respect the dignity and privacy of individual patients. Assist new employees in following facility processes, policies, and procedures. Assist patients with activities of daily living as documented in the patients’ care plan. Ensure patients have call lights at hand and answer call lights promptly. Ensure patient room is ready for receiving patient upon admission. Greet patient and ensure their comfort. Inventory and mark personal possessions. Assist with packing when patient is transferred or discharged Understanding and utilization of replacement of restorative devices and equipment. Education/Requirements: Must have completed an approved nursing assistant program and must possess a current, unencumbered, active certification. Must possess and maintain current CPR certification or acquire a CPR certification within 90 days of hire. The physical demands of this position include walking, sitting, repetitive movement, stooping, lifting 30 pounds, and may include assisting in the transfer of residents with mechanical assistance, transferring residents to/from bed/chair.
