Certified Nursing Assistant (CNA) Job Description
Certified Nursing Assistant “CNA” will act as a “Traveling Nurse” on a part-time basis, assisting isolated and vulnerable older adults who are aging in place and receiving services through the Office on Aging as authorized by the DHCD during the pandemic in Calvert County, MD. Services shall be provided by the CNA(s) 20-30 hours per week, not to exceed 140 hours per month. QUALIFICATIONS: 1. Hold a Maryland License in Nursing. Acceptable licenses are Certified Nursing Assistant (CNA), Registered Nurse (RN), or Licensed Practical Nurse (LNP). 2. Be able to work independently. 3. Be flexible to address client’s needs, schedule, and coordinate with staff. 4. Possess the ability to establish effective communication with clients, families, and team members. 5. Have knowledge of health and human resources for older adults. Promote senior resources and services. 6. Have the ability to use a smart phone, computer, and associated software programs. 7. Current CPR/First Aid Certification 8. Clear and positive criminal background check. JOB RESPONSIBILITIES: 1. Serve to increase health care access to isolated and vulnerable clients in Calvert County, Maryland. 2. Make home visits throughout Calvert County. 3. Be able to physically work with clients to provide an assessment of their medical needs. 4. Document, observe, and assess any changes in client’s condition. 5. Collaborate with team members to facilitate positive client outcomes. 6. Provide assessments to clients with a variety of physical and/or behavioral health issues. 7. Establish good relationships with clients and develop solutions to meet their needs. 8. Educate clients and their families about management of their illness(es). 9. Communicate with client’s health care provider as needed. 10. Provide information for statistical reports, if requested. 11. Attending meetings, seminars, trainings, and conferences, as needed. 12. Collaborate with the Behavioral Health Department and other relevant agencies, regarding client’s mental and behavioral health needs. 13. Communicate any client issues, scheduling conflicts/availability and documentation requirements directly to the Office on Aging, as well as with Loving Care Senior Services administrative office. 14. No medications are to be administered. 15. Shall perform simple nursing care related procedures as required, including: a) Taking accurate vital signs b) Wound care, as needed. c) Range of motion exercises or assistance with ambulation d) Positioning patient in bed (with proper body alignment) e) Bathing, shampooing, skincare, nailcare, oral hygiene, shaving and or dressing, as needed.
