Seeking LVN for residential care facility

A and F Care Services Inc. DBA Pacific Home ICF/DD-N is seeking to hire a full time Licensed Vocational Nurse for an Intermediate Care Facility. The facility caters to 6 adults with intellectual disabilities who require various skilled nursing needs. The facility is located in San Mateo, CA. Provide necessary nursing care to individuals residing at A and F Care Services Inc. nursing facilities within the scope of practice for a licensed vocational nurse. • Shall work closely with the RN Consultant to ensure the Health Care Plan is revised as necessary. • Shall ensure consumer’s rights are implemented. • The licensed vocational nurse will assist and/or provide medication administration to all individuals residing in the facility and review all medication documentation and recordings for compliance with the regulatory requirements and acceptable standards, no less than every two weeks (Semi-monthly drug regimen review). Such review will include documentation in the unit client record with specific notation of all noncompliances found and the corrective action taken. The corrective actions shall include a plan designed to prevent such errors in the future. • Review and revise as necessary, of the nursing services section of the individual service plan at least every six (6) months. based on visits and observations. • Shall report any abuse in the facility. • Notify the attending physician immediately of any sudden and/or marked adverse change in signs, symptoms or behaviors exhibited by the client. • Shall have the authority to make judgments regarding client-health issues within the scope of Nursing Practice Act. • Will apprise the administrator of recommendations, plans for implementation and continuing assessment through dated and signed reports which shall be retained by the administrator for follow-up action. • Assist staff with performing daily activities for clients. • Shall report any abuse or suspected abuse in the facility. • Shall be capable of working an eight (8) hour shift for any of the following shifts (AM shift: 7AM-3PM, PM shift: 3PM-11PM, or NOC shift: 11PM-7AM) or other depending on facility needs.
