SAYRE HEALTH CARE CENTER POSITION DESCRIPTION Position Title: Nursing Assistant Supervisor’s Title: Charge Nurse General Statement of Responsibilities: The Nursing Assistant is responsible for providing nursing care to resident in the areas of: daily hygiene, toileting, feeding, moving safely to activities and other functions, and maintaining the cleanliness and care of the resident’s room. Bona Fide Occupational Requirement: Current certification as a Nursing Assistant in the Commonwealth of Pennsylvania; or documentation of passage of the competency evaluation for a certified Nursing Assistant. Major Job Duties: 1. Assist residents with daily hygiene to include: bathing/showering, shampooing and combing hair, shaving, oral/teeth-denture cleaning, nail care, and applying lotions. 2. Assist resident in dressing i.e. dressed for the day, dressed to go out, dressed for bedtime. 3. Assist residents in toileting as necessary, empty and clean bedpans and urinals. 4. Assist residents at mealtime to include feeding assistance as necessary and report resident’s food intake and output as requested as well as any significant change in resident’s eating habit. Documentation of food and fluid intake on flow record. 5. Fill resident’s water pitchers and make sure glasses are available which are cleaned and sanitized. 6. Assist residents: in and out of bed or chairs as necessary, in accessing various areas of the facility, getting to meals, activities, and other functions. Use wheelchair when necessary. 7. Assist in the cleanliness of the resident’s room: wipe and clean over bed tables, change linens on beds; clean and organize resident’s drawers and closets, ensure residents have sufficient supplies. 8. Assist in the care and handling of resident’s laundry and ensue resident’s laundry is properly folded and put away in resident’s room. 9. Respond to resident’s call lights or buzzer promptly. 10. Immediately report any resident incident, noticeable change in condition to the Director of Nursing or Charge Nurse. 11. Assist in the straightening and in the cleanliness and neatness of the linen storage area. 12. Project a friendly, helpful, caring, and enthusiastic attitude to residents and their families. 13. Follow all resident care plans and approaches. 14. Ensure documentation is complete and accurate prior to end of shift. 15. Perform other duties as assigned by the Director of Nursing, the Charge/Registered Nurse, or Administrator related to the resident care or overall maintenance of the facility operations. Knowledge, Skills, & Abilities In: General nursing assistance practices and techniques such toileting, bathing, showering, proper, and safe lifting techniques; general behaviors and traits of geriatric population and how to work effectively; ability to read and write, ability to follow written and verbal directions; ability to work as a treatment/health team member; ability to show interest and care to residents and their families; knowledge of: sanitation, universal precautions and safety; ability to work varied schedules to include weekend shifts, and be prompt and consistent in maintaining work schedule. Physical Requirements: Must be able to physically assist residents in toileting, showering, bathing, walking; must be able to physically assist residents in and out of wheelchairs, in and out of bed, in and out of chairs, etc., must be able to physically assist residents in walking and mobility; must be able to physically walk and move about the facility; must be able to stand and walk/move about the facility for an eight hour shift, must be able to bend and stoop. Must be capable of exerting 50-100 lbs. of force occasionally (less than 1/3 of the time), and/or excess of 25-50 lbs. of force frequently and up to 10-25 lbs. constantly; must be able to move objects, equipment, and residents.
