Licensed Practical Nurse (LPN) Job Description
LPN position Full time Evenings 3pm-11pm! Job Summary: Provides care to residents in accordance with physician orders, recognized standards of practice and established company policies and procedures. Essential Functions Assists in maintaining a physical, social and psychological environment in the best interests of residents. Ensures resident rights and confidentiality are maintained at all times. Monitors residents within state licensure regulations; supervises and directs nursing assistants where allowed by state licensure regulations. Assists on writing care plans based on resident needs and change of condition. Obtains and administers medication and treatments as prescribed by physician. Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations. (Including pharmacy orders, and checking in medications). Monitors responses to care treatment plans. Possesses knowledge of medication interactions and notifies Nursing Supervisor of any issues. Adheres to company systems, processes and procedures. Counts controlled substances with off going nurse or designee; secures and corrects reconciliation of controlled substances during shift. Monthly Skin Assessments. Ensures family members are aware of residents need for scheduled medical, dental, or eye appointments. Ensures family members are aware of residents change of condition. Schedule transportation for residents to physician appointments when need on designated days. Ensures cleanliness and organization of medicine supply room and security of discontinued medicine in accordance with federal and state regulations. (Including cleaning, stocking supplies, setting up the nursing carts). Assists with discharge process relevant to medication and treatment orders. Complete New Admissions (assists with admissions from prior shift when applicable). Taking responsibilities for the assigned Mars and Tars Monthly. Filing when needed. Possesses knowledge of OSHA regulations, infection control standards and emergency procedures. Follows policies and procedures to ensure resident safety. Documents all pertinent information regarding nursing care, care plans and observation of resident overall condition. Attends training to maintain skill level and remain current in emerging healthcare trends. Supervises staff schedules during call off to staff according to facility requirements. Supervises and educates resident care staff to ensure duties are performed. *This job description represents an overview of the responsibilities for a LPN. Education / Experience / Certifications: State nursing license (LPN, LVN), Basic CPR or first aid training and fire safety training are also required. Management / Decision Making: Makes decisions based on existing guidelines and procedures. Solves problems using standard procedures and precedents, knowing when to refer issue to supervisor. Knowledge and Skills: Possesses a thorough understanding of the organization and work environment. Familiarity with Microsoft Windows, Office and Outlook email preferred. Basic typing skills are essential as well as having the ability to maintain resident confidentiality.
