Home Healthcare Licensed Practical Nurse - Springfield, OH

The LPN/LVN provides nursing care to individuals who require specialized knowledge, judgement and skill derived from the principles of biological, physical, behavioral, social, and nursing sciences in the client's home. Nursing care is performed in accordance with a licensed physician’s orders under the direction and supervision of the Director of Nursing and the RN Clinical Manager. Qualifications: • Licensed Registered Nurse in the state of Ohio. • Preferred minimum of one (1) year of experience in home health care. • Valid state driver’s license. • Current CPR certification. • Ability to perform essential functions of the job such as visual and auditory assessments of the client. • Ability to read and interpret medical instructions related to the client. • Ability to prioritize, reason and effectively communicate with all levels of the health member team, including the client and the client’s representative(s). Responsibilities: • Provides one on one care to the client in their homes while participating in the planning and coordination of total patient care in conjunction with the RN Clinical Manager and under the direction of a licensed physician’s plan of treatment. • Conducts a health status assessment of the client, including vital signs, physical assessment, mental status, observation and interview by collecting data for the purpose of providing nursing care. • Observes signs and symptoms and reports to the physician reactions to treatments, including drugs, and changes in the client’s physical, behavioral, social or emotional condition. • Educates, teaches and counsels the client and/or client representative(s) regarding the nursing care needs and other related issues of the client in the home. • Documents the client’s condition and care, including signs and symptoms, which may be indicative of change in an appropriate and accurate manner. • Reports any changes in the client’s condition, environment or any client related incidents/complaints to the RN Clinical Manager in a timely manner. • Consults with the attending physician concerning alteration of the plan of treatment in coordination with the RN Clinical Manager. • Processes changes in physician orders as needed. • Maintains current Medication Administration Record (MAR). • Participates in case conferences and staff development meetings.
