Seek direction/inform Supervisor if directed to perform a duty you have not been trained to complete or feel uncomfortable. 4. Respect Resident’s Rights. 5. Ask questions. Report to work as scheduled. Do not leave shift unless excused by supervisor. Wear neat, clean, pressed uniform. Maintain personnel hygiene and grooming. Avoid perfume, long nails, and excessive jewelry. Show respect for Supervisor and all other staff. Cooperative and accepts assignments willingly. Organize and complete assignments in time allotted. Leave unit only for allowed breaks and meal times. Returns after 15 minutes (breaks) and 30 minutes (meal time) if working more than a four hour shift. Sign out and in at Nurses station. Always break opposite your “wing partner”. Phone calls made/received only during break times or with Supervisor’s permission. Assigned residents are clean, neat, appropriately dressed. a.) Clean/good hygiene – proper oral care, peri – care. b.) Hair fixed. c.) Facial hair removed per patient preference. d.) Nail care complete (at least every bath day fingers and toe nails trimmed UNLESS diabetic per care plan.) Assist Residents in achieving hydration/nutrition per care plan. Observant – report all changes in condition to Supervisor. Resident call – lights a.) Always leave within resident’s reach. b.) Respond to call – lights of all Residents (not limited to assigned group) in a timely manor. Checks Resident daily schedule to ensure dressed appropriately and on time for activities, therapy and physician appointments. Complete vital signs, weights, equipment sanitization and other assigned duties. Deliver restorative care per care plan. a.) Turn and reposition every 2 hours with proper postural supports. b.) Adaptive devices. c.) ROM. d.) Promote ADL skills. e.) Achieve / maintain maximum mobility – e.g. ambulation. f.) Continence program – toileting schedule. g.) Notify Supervisor if Resident declines. Maintain Resident and Staff safety at all times. Use kind, gentle approach with Residents. Always respect Resident Rights. (e.g. dignity, privacy). (A positive, cheerful attitude is contagious – smile.) Handles difficult situations appropriately. Know when to seek assistance. Complete documentation: weights, vital signs, I&O, report book, mobility records, and sign off care plans. If resident receiving oxygen (O2) Therapy: follow care plan and policy. Tubing off the floor. Signs placed on door and above bed. Ward order complete. Resident room clean and neat. Attend / complete scheduled in-services and strive to improve, knowledge and skills. Maintain positive attitude and promote team work. a.) Work effectively with peers. b.) Respect Supervisors and supports decisions. c.) Help others willingly after own work is completed. d.) Offer ideas to improve care. Familiar with emergency procedures and responds effectively to e.g. fire drills. Compliant with facility policies. Assist with orientation of new employees. Other duties as assigned.
