Licensed Practical Nurse (LPN) Job Description
Helping Hands, LLC is looking for reliable LPN's to come aboard part-time/prn for our home nursing agency. 1) The licensed practical nurse may perform selected acts in accordance with the Nurse Practice Act and under the direction of an RN, including administering treatments and medications in the care of the ill, injured or infirm; health maintenance; and illness prevention. 2) The licensed practical nurse shall report changes in the patient's condition to the RN, and these reports shall be documented in the clinical notes. 3) The licensed practical nurse shall prepare clinical notes for the clinical record. Licensed Practical Nurse Qualifications/Skills: ● Critical Clinical Thinking Skills ● Excellent Bedside manner ● Infection control advocate ● Physiological knowledge ● Medication knowledge ● Medical teamwork ● Multitasking, listening, and verbal communication ● Health promotion and maintenance Education, Experience, and Licensing Requirements: ● LPN Illinois License ● Clean background check and drug screen ● Driver's License and Insurance ● Current CPR/BLS Recognize the needs and behaviors of a variety of age groups of patients treated, needs and behaviors of patients with Alzheimer's and Dementia. Written and verbal competency of the English language. Must be able to travel to all of the service office boundaries in all extremes of weather on all types of roads including expressways. Must be able to assist with ambulation including turning and transferring patients up to 200 pounds. Required to make visits to private residences which may not be accessible to people with disabilities. If position has direct patient care or direct patient contact the following lifting requirement: Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when a patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis. Purpose: The purpose of this position is to help achieve the vision of Helping Hands Home nursing services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Interdisciplinary team and as such meeting individual performance expectations and contribution to team performance goals is required.
