Home Health Aide (HHA) Job Description
Helping Hands is looking for reliable caregivers for our clients . Certified Nursing Assistant/Home Health Aide 1) Responsibilities/ services shall be under the supervision of an RN in accordance with the plan of treatment. The RN shall assign the home health aide to a particular patient. The RN or the appropriate therapist shall prepare written instructions for patient care. 2) Duties of the home health aide and/or CNA may include: A) Performing simple procedures as an extension of therapeutic services; B) Skilled personal care and personal care, as defined in this Part; C) Patient ambulation and exercise; D) Household services essential to health care at home; E) Assisting with medications that are ordinarily self-administered; F) Reporting changes in the patient's or client's condition and needs to the RN or the appropriate therapist; and G) Completing appropriate records. Job Responsibilities: ● Providing great patient care. ● Answering patient calls and determining how best to help them. ● Providing physical support for patients or clients with daily activities and personal hygiene, including bathing, dressing, getting out of bed, - using the toilet, walking, standing, or exercising. ● Turning and repositioning bedridden patients. ● Ensuring patients or clients receive an appropriate diet by reviewing their dietary restrictions, food allergies, and preferences. ● Obtaining a wide range of information from physicians, caregivers, and nurses about patient conditions, treatment plans, and suggested activities. ● Measuring and recording food and liquid intake and urinary and fecal output and reporting changes to medical or nursing staff. ● Recording vital signs, including blood pressure, pulse, temperature, and respiration rate as requested by the agency as part of the Client plan of care, under supervision of RN. ● Examining patients to detect issues requiring medical care, including open wounds, bruises, or blood in the urine. ● Reminding patients to take medications and nutritional supplements. ● Noting observations of patient behavior, including complaints, or physical symptoms to nurses. ● Stay up to date on CNA/HHA training and Agency policy and procedure. HHA provides assistance with activities of daily living, housekeeping, personal laundry, and companionship provided to an individual in his or her personal residence, which are intended to enable that individual to remain safely and comfortably in his or her own personal residence. Position Requirements: Recognize the needs and behaviors of a variety of age groups of patients treated, needs and behaviors of patients with Alzheimers and Dementia. Written and verbal competency of the English language. Must have a home health aide certificate in the State of Illinois Or CNA Certificate of IL. Must be able to travel to all of the service office boundaries in all extremes of weather on all types of road including expressways. Must be able to assist with ambulation including turning and transferring patients up to 200 pounds. Required to make visits to private residences which may not be accessible to people with disabilities. If position has direct patient care or direct patient contact the following lifting requirement: Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when a patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.
