Certified Nursing Assistant (CNA) Job Description
Objectives: It is the goal of Pioneer Nursing Home that each resident receive the necessary care and services to attain or maintain the highest possible physical, mental, and psychosocial well-being in accordance with this facility’s Mission Statement: “As staff members of Pioneer Nursing Home, our mission is to provide diligent professional and compassionate care to all residents with a focus on individual needs, dignity, self-determination and respect. The NA/CNA is the person directly responsible for the hands-on care of our residents to help maintain the highest level of function spiritually, mentally, physically, and emotionally possible without a decline in status unless decline is documented as unavoidable. 2. The NA/CNA works under the direction of the shift charge nurse. 3. The NA/CNA is under the supervision of the Residential Care Manager to follow the individualized plan of care written for each resident. 4. The NA/CNA functions as part of the Nursing team and works in compliance with Pioneer Place Policies & Procedures, State and Federal Regulations. Qualifications, Skills, Knowledge: 1. Must be 18 years of age or older. 2. Must have completed an approved nursing assistant program. Must pass skills check, written test and receive certification with-in 120 days of employment as a Certified Nurse’s Assistant in the state of Oregon. 3. Enjoys working with the elderly, people with disabilities, or those who need rehabilitation, and is sensitive to their needs. 4. Ability to work well with others, maintaining good communication. 5. Required to be in good physical and mental health. a. Physical activities may include being able to perform the following: i. Moving, transferring, and lifting (75-100 pounds) ii. Walking iii. Repetitive bending iv. Pushing and pulling v. Standing vi. Kneeling, crouching, stooping vii. Reaching. viii. Sitting 6. Must be able to read, write, and speak English and do simple mathematics. 7. Must be able to see and hear well enough to make observations and report to Charge Nurse any observed changes in resident condition. (100 % of the time) 8. Understand policy and procedures in regard to exposure to blood/bodily fluids, fire and evacuation, and good resident care practices. Attends and participates in facility In-service and trainings. 9. Demonstrate proper technique operating mechanical lifts, wheelchairs, beds, and other medical devices. 10. Must have some computer skills and be willing to learn computer documentation required on electronic medical record. 11. Required to maintain CPR certification BLS or CPR for Healthcare worker. 12. Compliant with HIPAA regulations, maintains strict confidentiality of client information. 13. Complies with Infection Control, Standard Precautions, and OSHA standards for Healthcare Professionals. 14. Health Requirements: In good health and able to work without restrictions. 15. Compliance with Supplemental Requirements such as TB screening, vaccination records, HR & Clinical Orientation dates, and miscellaneous in-service dates. Standards of Care: 1. Every resident, regardless of functional and cognitive ability will be appropriately dressed, clean and well-groomed every day. 2. Activities of Daily Living include: a) Transfers b) Bed Mobility c) Toileting (Bedpan, urinal, Commode and/or toilet) d) Skin Care/Hygiene ( face washing, Perineal care, cleansing of skin, lotion, pressure ulcer prevention) e) Bathing (Bed bath, showers, tub bathing) f) Oral hygiene/denture care g) Dressing/Undressing h) Meal Assistance (Setup, feeding assist) 3. Restorative care to residents daily, which may include but limited to: ambulation, positioning, toileting, active ROM,
