Home Health Aide (HHA) Job Description
DUTIES:
The hospice aide is responsible for providing personal care under the direction of a
registered nurse.
RESPONSIBILITIES:
1) Perform personal care (bathing, dressing, toileting, feeding, etc.) according to the Hospice Aide Plan of Care.
2) Document care provided according to agency policy.
3) Report any deviations from the Plan of Care to the Case Manager (or designee if not available).
4) Use Durable Medical Equipment safely.
5) Take vital signs when on Hospice Aide Plan of Care.
6) Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
7) Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager.
8) Assist with ambulation, transfers, range of motion if documented on the Hospice Aide Plan of Care.
9) Provide respite for patient’s family/caregiver when on-site, as appropriate.
Participate in QAPI activities.
10) Participate in IDG meetings, as appropriate.
QUALIFICATIONS:
1) Meets training requirements of the State.
2) Received a minimum of 75 hours of classroom and supervised practical training with at least 16 hours devoted to supervised practical training and with at least 16 hours of classroom instruction completed.
3) One year experience as a CNA, hospice aide or Hospice aide. (Hospice preferred)
4) At least 18 years old.
5) Must be a licensed driver with an insured automobile in good working order.
6) The ability to communicate well, both verbally and in writing.
Compensation on a per diem (per visit) basis, $35 to $40 per visit, depending on experience and travel distance.
