Essential Functions: Assist, perform, and train patient/client and family in activities of daily living such as bathing groom, toileting, and adequate Nutrional intake. Assist as assigned with ambulation and transfers. Maintain a clean, safe, and healthy patient/client environment. Assist patient/client assignment with incidental activities of daily living such as shopping, meal preparation, socialization activities, homemaking, and medication reminders. Utilize basic infection control measures such as hand washing, and personal protective equipment (PPE) appropriate to the task and assignment. Use proper body mechanics. Engage in respectful social interactions with the patient/client, including friendly conversation and empathic support as well as respecting client's privacy and property. Exhibit positive attitude, behavior, demonstrate respect to other employees, clients maintain confidentially of all information pertaining to the patients/clients and their families. Organizes self to carry out visits/shift and tasks. Attend mandatory company in service and provide requested documents to keep employee files current. Follow the assignment sheet/ service plan performing only task that are assigned. Document and report and noticeable changes in patient/client physical condition, attitude, reaction, appetite, etc., to the supervisor/manager in a timely manner. Report all client and employee incidents/accidents to your supervisors/manager immediately. Complete other assignments as requited and assigned. May have access to personal health information (PHI) necessary to fulfill the above duties and responsibilities.
