We serve those who are in the final stages of their journey while honoring their life. We are searching for experienced and caring CHHAs to join our team. The Certified Home Health aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse providing various assistance to care for the patient’s personal needs and to promote comfort. The CHHA is critical in the role of observing the patient, reporting those observations and documenting and providing the highest level of quality care. Position Role and Responsibilities · Operating as the “Eyes and Ears” of the Care Team · Performing personal care needs as determined by the approved Plan of Care and attending to his/her requests promptly; working with RNs on POC changes as needed · Providing all needed activities of daily living and medication reminders, assisting in ambulation and exercise as instructed · Providing proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence · Utilization and timely submission of the company’s EMR to document on each shift’s assigned plan of care duties at the end of each shift · Communicating and reporting on patient’s condition and significant changes to the RN/Case Manager and/or Director of Patient Care Services Qualifications/ Position Requirements · Current, unrestricted certificate as a Home Health Aide in the State of California · Complies with accepted professional standards and practice · Demonstrates good verbal and written communication in English, and organization skills · Comfortable with any cloud technology and experience with Electronic Medical Record software · Active CPR certification and verification of negative TB skin test or clear chest Xray · Must be a licensed CA driver with access to an automobile that is insured in accordance with state/or organization requirements and is in good working order for self-transportation to all work shifts American Veterans Hospice is locally operated and owned by a Veteran.
