OVERALL JOB PURPOSE The overall purpose of the Wellness Nurse position is to provide Person-Centered Care consistent with accepted standards and expectations of Sunshine Retirement Living and state regulations. The Wellness Nurse responsibilities include, but are not limited to, monitoring resident health status, providing physical care, early diagnosis and intervention of resident changes in health status. Wellness Nurses must communicate politely and effectively with family, primary physicians and perform all administrative charting or other services as required by regulations and company policy. MINIMUM JOB QUALIFICATIONS AND REQUIREMENTS A current, valid LPN or RN license is required Education/and or experience in long-term care, acute care, or geriatric nursing preferred Supervisory experience preferred Ability to read, write, and speak the English language and to verbally communicate with different levels of staff, families, and any outside customers in a professional and polite manner No disqualifying criminal or care offenses as defined by regulatory guidelines Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations Ability to work under time constraints and meet deadlines Maintain a positive and respectful approach with coworkers. Ability to work in a team setting and be a team player Ability to write nursing/patient progress notes, and nursing procedures Ability to effectively present information and respond to questions from department heads and customers(patients, family members, physicians, etc.)in a polite and professional manner Satisfactorily pass Sunshine’s and state-regulated criminal record clearance, health screening, and TB clearance (if applicable) ESSENTIAL JOB DUTIES AND RESPONSIBILITIES Implements and evaluates resident’s plan of care Identifies and secures equipment and supplies. Notifies supervisor when supplies are needed Ensures appropriate and timely response to resident and/or family concerns Responsible for the safety of residents under your supervision Informs appropriate staff about the condition of residents and expectations/needs for the shift assigned at the beginning of each shift and receives report from coworkers throughout and at the end of each shift regarding resident conditions Directs Caregivers to assure care provided per standards of practice and per community policies and regulations Rounds with Caregiver prior to end of shift to assure unit is in proper order Performs other duties as assigned Dedicated to delivering a high level of customer service Provides resident care in accordance with accepted standards of practice and within the scope of the LPN or RN license Routinely observes and reports on resident’s conditions/changes and then documents in accordance
