Licensed Practical Nurse (LPN) Job Description
As a Licensed Practical Nurse (LPN), you will play a vital role in delivering compassionate and high-quality resident care in the long-term care setting. You will work closely with registered nurses and physicians to provide essential support, ensuring that residents receive the best possible care. Your expertise in nursing practices will be instrumental in enhancing the well-being of our patients, particularly in long-term care, and memory care.
What you’ll do
• Administer medications and treatments as prescribed, ensuring accurate dosage and adherence to safety protocols.
• Conduct patient assessments, monitoring vital signs and documenting changes in health status.
• Provide direct patient care, including assistance with daily living activities and wound care.
• Maintain accurate medical records and documentation in compliance with HIPAA regulations.
• Support patients with developmental disabilities, Alzheimer’s, memory care issues, and geriatric needs.
Basic qualifications
• Valid LPN license from an accredited nursing program
• Strong understanding of medical terminology and nursing practices
• Proficient in using Electronic Health Record (EHR) systems such as PCC
Preferred qualifications
• Background in long-term care facilities or assisted living environments
• Familiarity with ICD-9 and ICD-10 coding for accurate medical documentation
• Knowledge of Alzheimer's care, memory care, and geriatric care
• Experience working with diverse populations including geriatrics
