Health Care Coordinator

Strong, reliable LPN wanted to manage healthcare operations in an Assisted Living environment. Must have a good knowledge base of healthcare needs of the geriatric population, and excellent communication and organizational skills. Must be reliable, flexible, and willing to rotate weekend and evening call when needed. Managerial experience is preferred. Duties to include assessment, compilation, and implementation of service plans of residents using the EMR system, interfacing and communicating with residents, families, third party providers and physicians daily. Must have excellent assessment skills and the ability to triage severity of falls and other medical conditions to determine appropriate referral for treatment. Responsible for scheduling, managing, educating, and supporting healthcare staff. Staffing and scheduling for all positions of the healthcare department is one of the most critical components of the job. In the challenging labor market that we are experiencing, this job duty requires creativity and calm decision making under pressure.
