Seeking Licensed Practical Nurse to join our Home Health Care team!

Full-Time Licensed Practical Nurse (LPN): • Shift: Dayshift, Monday – Friday (40 Hours/Week) • Pay Rate: $28.00 - $31.00 per Hour • Location: Columbus, Ohio • Available Territories: Central Columbus, Southeast Columbus & Southwest Columbus • Expected Travel: 30 Mile Radius (Mileage Reimbursement, $0.48/Mile) Responsibilities: • Provide direct patient care under the supervision of a registered nurse. • Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes. • Implement plan of care initiated by the registered nurse. • Provide accurate and timely documentation consistent with the plan of care. • Assesses and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care. • Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager. • Use and/or prepare equipment and supplies effectively and efficiently adhering to aseptic technique. • Participate in personal and professional growth and development. • Perform other duties assigned by the registered nurse. Qualifications: • Graduate of an accredited practical nurse or vocational nursing program. • Must hold a current license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio. • Current licensure in state and CPR certification. • Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements. • Recent TB test, required. • 3 years of nursing experience, preferred. • Prior experience in community/home health and/or or medical/surgical, preferred. • Comply with accepted professional standards and practices. • Demonstrates good verbal and written communication, and organization skills. • Must be able to lift, position or transfer patients and lift supplies and equipment.
