Southeastern Healthcare is seeking to fill the role of a Licensed Practical Nurse (LPN) Home Assessment Nurse This individual plays a crucial role in assessing and providing healthcare services to patients in their homes. Job Responsibilities include: Patient Assessment: • Conduct thorough assessments of patients' health status in a home setting. • Evaluate patients' medical histories, current conditions, and medication regimens. Care Planning: • Develop individualized care plans based on the assessment findings. • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care. Monitoring and Observation: • Monitor patients' vital signs and overall health status during home visits. • Observe and document changes in patients' conditions. Medication Management: • Administer medications as prescribed by healthcare providers. • Educate patients and their families about proper medication administration and potential side effects. Patient Education: • Educate patients and their families on managing chronic conditions and promoting overall wellness. • Offer guidance on lifestyle modifications and health maintenance. Communication: • Communicate effectively with patients, their families, and other healthcare team members. • Provide clear and concise documentation of assessments, interventions, and patient responses Qualifications: Education: • Current and valid LPN license in the state of Georgia. Experience: • Previous experience in home healthcare or a related field is preferred. Skills: • Strong clinical assessment and critical thinking skills. • Excellent communication and interpersonal abilities. • Proficient in basic nursing procedures and techniques.
