Licensed Practical Nurse (LPN) Job Description
Job Summary: We are seeking a skilled and compassionate Registered Nurse to join our home care team. The RN will provide direct nursing care to patients in their homes, oversee care plans, and supervise caregivers to ensure high-quality, patient-centered care. This role requires strong clinical judgment, excellent communication skills, and a commitment to improving the health and comfort of patients in a home setting. Responsibilities: Conduct initial and ongoing patient assessments in the home setting. Develop, implement, and update individualized care plans. Administer medications, treatments, and skilled nursing procedures as prescribed. Monitor patient progress and report changes in condition to physicians and families. Provide education and support to patients and their families regarding health conditions and care needs. Supervise and guide caregivers, CNAs, and home health aides to ensure care quality. Maintain accurate, timely, and complete clinical documentation in accordance with agency policies and state regulations. Coordinate care with physicians, therapists, and other healthcare professionals. Ensure compliance with infection control, safety, and quality standards. Qualifications: Current and valid Registered Nurse (RN) license in the state of [insert state]. Minimum of [1–2 years] of nursing experience, preferably in home health, acute care, or long-term care.
