Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients. We are seeking a compassionate, skilled LPN or RN to provide one-on-one nursing care for a 13-year-old child with Spastic Cerebral Palsy in a home setting. The ideal candidate is patient, attentive, and experienced in pediatric and/or neurological care, with a strong commitment to supporting the child’s medical needs, comfort, safety, and overall quality of life.

Pay: LPN hourly rate $35 - RN hourly rate $45
Schedule: 1 day - Sunday 12 hr shift

Responsibilities
- Provide direct nursing care in accordance with the child’s individualized care plan
- Assist with activities of daily living (ADLs), including positioning, mobility support, hygiene, feeding, and toileting as needed
- Monitor vital signs, neurological status, muscle tone, and overall health
- Administer medications and treatments as prescribed
- Perform range-of-motion exercises and support physical therapy goals as directed
- Manage and monitor medical equipment (e.g., feeding tubes, suctioning equipment, braces, mobility devices, if applicable)
- Recognize and respond appropriately to changes in condition or medical emergencies
- Maintain accurate and timely documentation of care provided
- Communicate effectively with parents/guardians, therapists, physicians, and care team members
- Provide emotional support and promote a nurturing, respectful environment for the child
- Follow infection control, safety, and privacy standards at all times
Qualifications
- Current, valid LPN or RN license in good standing
- Experience with pediatric patients and/or neuromuscular or developmental disabilities preferred
- Knowledge of cerebral palsy and related care needs is a strong plus
- CPR/BLS certification (pediatric preferred)
- Ability to safely lift, transfer, and position a child with mobility limitations
- Strong assessment, critical-thinking, and communication skills
