LPN-Afternoon Supervisor

St. Anne's Mead is hiring a full-time Afternoon Supervisor for its Assisted Living & Memory Care Home. This position will work closely with the Director of Nursing to supervise direct-care staff and provide nursing oversight for the residents in Assisted Living and Memory Care. St. Anne's Mead is a licensed Assisted Living (Home for the Aged) with a dedicated Memory Care Unit. We want to hire the best of the best, creating an awesome work environment for staff and living environment for our residents. We do very important work in caring for elderly adults and it is extremely rewarding to do it well. Responsibilities of this position includes, but not limited to: Together with the DON, supervise the direct care staff working a 3pm - 11 pm shift, providing education and direction as needed. Ensure that the individual service plan is followed for all residents; administer or supervise treatments prescribed by physician. Make meaningful rounds to all residents, daily to ensure that all are well cared for. Receive or place calls to physicians and follow through with physician's orders. Timely and accurately prepare incident reports. Communicate with residents' families regarding incidents, accidents, medication changes etc. ** Candidate Must be able to provide proof of COVID-19 Vaccine or willing to get Vaccinated prior to hire**
