Wellness Nurse/LPN

The Wellness Nurse/LPN is responsible for performing a variety of duties to provide quality nursing care to residents and to coordinate total nursing care for residents; implementing programs and specific procedures; Working closely with the Wellness Director in establishing specific goals. Perform various duties to provide quality nursing care to all residents Assist residents and obtain information for appropriate interventions to prevent decline; notify appropriate staff per facility protocol, record weight loss, record signs of change in condition, record any swallowing issues, decline in ambulation, report of skin breakdown, etc.; observe conditions which indicate possible need for restorative nursing programs and then notify appropriate staff per facility protocol of the need for evaluation; inform the physician of changes in assessment when appropriate. Administer medications and treatments to residents; place orders for medication from the pharmacy. Assist residents upon admission, Attend and participate at Plan of Care meetings as scheduled. Keep charts updated Make rounds to see each resident daily and as needed. Responsible for supervising nursing personnel to deliver nursing care and within scope of practice coordinates care delivery, which will ensure that residents' needs are met in accordance with professional standards of practice through physician orders, center policies and procedures, and federal, state and local guidelines. Qualifications: Graduate of an approved school of nursing Current state licensed practical nurse required Minimum of six (6) months experience working with seniors in assisted living, home health, independent living, hospital or long term care environment and desire to serve and care for seniors Demonstrates good judgment, problem solving and decision making skills We are looking to hire at part time and full time LPNS. We have 8 hour shifts available, but are also open to offering 12 hour shifts (three 12 hour shifts = Full Time).
