Licensed Practical Nurse (LPN) Job Description

Pay is based on experience. Wages starting at $23.00 per hour. Work hours are three, twelve hour shifts. Also, one weekend out of the month and every other holiday are to be scheduled for the shift. Responsible for organizing, planning and directing assisted care for Residents and making daily assignments and overseeing the job performance of the Nursing Assistants. Responsible for the safety and wellbeing of Residents and staff, and supervision of the entire facility when Director of Nursing and Administrator are not present. Maintain Rights of Residents as set forth by the Ohio Department of Health Laws and Regulations. Essential Duties and Responsibilities include the following: • Demonstrates a sincere, caring and respectful attitude to Residents at all times. • Utilizes good communication skills with Residents, families, and other staff members. • Maintains a professional, clean, and neat appearance following nurse dress code. • Plans and organizes Nursing Assistant daily assignments and activities as needed, and monitors and updates individual service plans to ensure Residents’ needs are met in accordance with physician orders and facility policies and procedures. • Observes nursing care and visits Residents to ensure that nursing care is carried out as directed and treatment administered in accordance with physician’s instructions. • Communicates with physicians, physicians’ offices, appropriate healthcare providers and agencies, and families about Resident problems and concerns, changes or symptoms. • Responsible for staffing and maintaining adequate coverage to provide quality care to Residents. • Responsible for completion of staff schedule exception records (i.e. call off’s tardiness, missed punches, etc.) • Documents on staff problems and works in conjunction with Director of Nursing on disciplinary counseling. • Ninety day and annual evaluations for nursing assistants as assigned by Director of Nursing. • Assists in preparation and maintenance of Residents’ clinical records. Admits, discharges and transfers Residents as needed. • Assists with ordering drugs and equipment, and maintaining narcotic records. • Receives report from previous shift nurses and gives the following shift nurse report before leaving at shift end. • Assists with administering medications to Residents as outlined by Company policy and procedure. • Documents all falls, injuries, problems, or pertinent information on Residents’ files. • Notifies the doctor and/or family in case of accidents or problems with any Resident. • Understands fire and disaster plans and reacts appropriately if an emergency arises. • Knowledgeable of and implements all safety, universal precautions, and Company policies/procedures for the protection of residents, coworkers, and self.
