Licensed Vocational Nurse (LVN) Job Description

Licensed Vocational nursing personnel shall perform, but not be limited to, the following: 1. Works directly under the supervision of RN, Clinical Supervisor or ADOPC. 2. Re-evaluates the patient’s nursing needs q visit/shift, updating the physician orders, medication profile, MAR & POC as required. 3. Prompt reporting to immediate supervisor of any and all incidents. Written report on appropriate form in agency within 24 hrs of event. 4. Inform the physician, assigned nursing personnel, Clinical Supervisor, ADOPC and family of changes in the patient’s condition and needs. 5. Initiate appropriate preventive and rehabilitative nursing procedures. 6. Counsel and instruct the patient, family, and other nursing personnel in meeting nursing and related needs. 7. Assume responsibility for professional growth by participating in in-service training programs and skills labs. 8. Follows infection control and safety measures at all times. 9. Adhere to the policies, procedures and standards of the facility to which assigned. 10. Follow all regulation guidelines, i.e.; mandatory reporting, Title 22, and Medicare CoP’s. Medi-Cal’s directives. 11. Complex medical treatments under physician’s order to include, but not limited to gastronomy, tracheostomy, and surgical aftercare. Quarterly training for vent/trach complex medical treatment. 12. Provides direct patient care, evaluates outcomes, consult with other practitioners as required and adjusts nursing care processes as appropriate to ensure optimal patient care under direction from the RN. 13. Provides personal care as ordered by MD and according to the HHA care plan created by the RN, PT, OT, or ST, and when a certified HHA is not available to provide the care. 14. Advocate for patient rights and identify potential conflict. Seek counsel for resolution of conflict. 15. Demonstrate competence to provide patient specific care which includes factors such as physical condition, diagnosis and its meaning to the patients. 16. Administer medications and treatments as prescribed by the physician. 17. Use supplies and equipment effectively and efficiently. 18. The LVN II performs venipuncture for lab draws, reports results to MD and initiate new orders if indicated by MD. 19. Observes and reports behavioral changes to case managers and physicians involved in POC. 20. Helps patients improve their coping abilities. 21. Builds relationships with patients to encourage trust, while listening to and interpreting their needs and concerns. 22. Responds to distressed patients in a non-threatening manner and attempts to understand the source of distress. 23. Applies “de-escalation” techniques to help patient manage their emotions and behaviors. 24. Gives report to Case Manager and consult when needed with the RN for guidance and direction. May consult with ADOPC/Clinical Supervisor if needed. 25. Demonstrate knowledge and awareness of legal issues in all aspects of patient care. 26. Provides escort/transportation services (Private Division only). 27. Prepares and submits timely documentation of patient’s care including Skilled Nursing Progress Notes, Supplemental Orders, and Case Conference Notes. Note: Submission of clinical documentation, Skilled Nursing Progress Notes and/or daily time summaries is required for satisfactory completion of an assignment as defined by agency policy. Failure to provide this documentation will result in disciplinary action. 28. Maintains confidentiality regarding patients, health care staff and documentation.

Compensation

$21.00 - $45.00 per hour + $3000 Sign-on Bonus
