Personal Care Assistant (PCA) Job Description
Job Description: Assists individual’s with activities of daily living such as eating, bathing, dressing, toileting, mobility, and transferring in and out of beds, chairs, and vehicles Remains awake and monitors the individual during sleeping hours Monitors health and well–being of consumers- reports signs of illness& health concerns promptly. Provides training according to the plan of care; training will include, but is not limited to: money management, cooking, shopping, socialization skills, recreation skills, etc. Carries out behavior management plan as outlined in the consumer’s plan of care Teaches/provides personal care and safety to consumer. Teaches the consumer skills to perform work and volunteer tasks in the community and skills that will help the consumer fulfill their medical needs. _ Provides transportation for consumer as necessary. Maintains daily progress notes and other records as instructed by the supervisor Collects data for the consumer’s plan of care, baseline evaluations, etc. as instructed by the supervisor Assists in the consumer’s program planning and revisions Attends all required in- service training sessions Promptly reports any incidents that occur involving the consumer, the vehicle, or any other property Follows policies and procedures concerning leaves of absences Respects the consumer’s right to confidentiality Reads and understands the consumer’s plan of care before working with them Maintains good communications between supervisors, consumers, and their families Performs other tasks as assigned and deemed necessary by the supervisor or his/her designee, for the care of the consumer according to policy and procedures and the consumer’s plan of care
