Registered Nurse (RN) Job Description
Unique Hospice is seeking a passionate and dedicated Registered Nurse (RN) to join our team. This position involves direct patient care in the field, focusing on individuals receiving end-of-life care. As part of our interdisciplinary team, you will play a crucial role in enhancing the quality of care and life for our patients. Responsibilities: Provide direct patient care as defined under the State Nurse Practice Act, with a focus on end-of-life care needs. Accurately document patient services to ensure they reflect the planned care and are compliant with legal and organizational standards. Assess patient and family/caregiver needs, providing education and information relevant to the diagnosis and care plan. Participate actively in the coordination of hospice services, reporting needs and adjustments to the interdisciplinary group. This includes attending biweekly interdisciplinary team meetings. Conduct RN supervisory and re-certification visits, ensuring timely and efficient care management. Maintain competency in nursing and assessment skills, identifying and communicating any changes in patient conditions that may affect the care plan. Fulfill other duties as assigned by the Registered Nurse or Case Managers. Qualifications: Must be a licensed driver with an automobile insured in accordance with State of California regulations. Strong understanding and embrace of hospice philosophy and the needs of terminally ill patients. Excellent prioritization and communication skills are essential.
