Registered Nurse (RN) Job Description
The position of Registered Nurse is accountable for the management of Divine Care In-Home Care Agency’s clients, through effective promotion, employment and administrative practices, assuring the quality and growth of all In-Home Care Services.
The position of Registered Nurse is accountable for maintaining all in-home care client care documentation, including Initial Screening and Evaluation of New Clients, In-Home Care Assessments, written Service Care Plans, Medication Profiles, Medication Administration Records, Supervisory Home Visits and Caregiver Orientation to Client Documentation. In addition, the Registered Nurse will also assist with New Caregiver Orientation and Training, as well as specific caregiver training related to the care of each client.
The position of Registered Nurse works with the Client Care Manager and HR / Staffing Manager who all report to the Administrator.
RESPONSIBILITIES
1. Complies with OAR 333-536-0000 through OAR 333-536-0125.
2. Provides leadership and guidance to Client Care Manager, HR / Staffing Manager, and all Caregivers.
3. Assists with new caregiver orientation and training and orientation to new In-Home Care assignments. Identifies caregiver continuing education and office staff development training needs.
4. Assists with planning in-service and continuing education programs.
5. Assists in the development of policies and procedures.
6. Monitors the quality of care provided through random record review, consultation with in-home care clients and/or client in-home visits.
7. Gives consultation in the office, or in the client’s home, as client care issues arise. Presents team conferences on a regular basis and as needed.
8. Reports client care problems, staffing problems and/or other disciplinary problems to the Administrator. Immediately reports any suspected abuse and/or neglect, or allegations of abuse and/or neglect, to the Administrator.
9. Administers In-Home Care client documentation, including Initial Screening and Evaluation of New Clients, In-Home Care Assessments, written Service Care Plans, Medication Profiles, Medication Administration Records, Supervisory Home Visits Documentation and Caregiver Orientation to Client Documentation.
10. Maintains communication with referral sources and community resources. Assists with development and implementation of marketing and/or promotional plans.
11. Must report to the Administrator and/or HR / Staffing Manager the employee’s arrest and/or conviction for a misdemeanor and/or felony crime within 10 days of the event. All employees who are licensed or certified by the Oregon State Board of Nursing, must also report to the Administrator any licensing board action that may prevent the employee from providing in-home care services,
12. Other responsibilities as directed by the Administrator. Other responsibilities may be assigned to this position in response to individual office needs.
DUTIES
NOTE: In addition to sitting at a desk, this position requires light physical effort and may require lifting, standing, bending, stooping, stretching, walking, pushing and/or pulling for short periods of time, with or without assistance from another health-care worker. Operating light office equipment, such as multi-line telephones, personal computers, copy machines and fax machines. Requires moderate to high degree of concentration due to volume, complexity and “pressure” of work (emergency situations). Clean working conditions with no disagreeable factors.
As needed, Monday through Friday - 9 AM to 5 PM - with time off for breaks and lunch.
Occasional after hours and weekend assignments, such as client care conferences, caregiver supervisory home visits, emergency in-home care service issues and/or other issues that may arise.
Occasional In-Home Care Supervisor On-Call - 24 hours per day - 7 days per week.
About once a month - Monday noon to Monday noon.
May be required to provide In-Home Care Services as an In-Home Care Provider in emergency situations when needed.
