1. Performs admission assessment within 24 hours referral or patients return from hospital 2. Assesses patients condition, initiated plan of care, re-evaluates as necessary. 3.Develops a written plan of care for each patient assigned. 4. Assesses and reassesses patients for pain. Utilizes appropriate pain management 5. Perform patient care responsibilities considering needs specific to the standard of care for patients age . 6. Follows the five (5) medication rights and reduces potential for medication errors. 7. Provides skilled nursing care. 8.Supervises the home health aide, or personal care aide as per regulations.
