Registered Nurse (RN) Job Description
Experienced Registered Nurse (RN or LPNS) Home Health -Weekday or Weekend Coverage
Seeking Bilingual nurses to see Spanish speaking or Chinese speaking patients.
Qualifications
- Current Illinois License and graduate of an approved school of nursing
- Must have prior home health care experience
- Ability to exercise initiative and independent judgement
Responsibilities
- Performs the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
- Initiates the plan of care and necessary revisions.
- Performs services in accordance with the plan of care.
- Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
- Coordinates services.
- Counsels the patient and family/significant others in meeting nursing and related needs.
- Participates in in-service programs.
- Processes orders and notifies physician of patient needs and changes in condition.
- Completes certification/recertification orders and discharge summaries.
- Refers to Physical Therapists, Speech Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
- Supervises and teaches other nursing personnel.
- Conducts patient care conferences on patient assigned to his/her care.
- Is available for on-call duty nights, weekends and holidays (not required but preferred)
We pay per visit upon completion of clinical charting - no hold periods for payroll.
Servicing non- COVID patients, COVID positive patients, and PUI cases
Chicago and surrounding suburbs
active license required
visit patients in their homes and follow care plan
ability to use EMR on phone a must - AXXESS experience is a plus!!!!
clinicians must use own transportation
charting knowledge per CMS requirements
Job Types: Part-time, Contract
Salary: $60 to $100.00 /hour
