Home Health Registered Nurse PRN

Monte Cristo HealthCare, Inc. is looking for a motivated RN that loves to provide excellent compassionate quality services. The ideal candidate is a team player, reliable, good communicator, compassionate, responsible, and a hard worker. Assess home health patients to identify the physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. Case Manage and provide clinical directions to the interdisciplinary team, physician and family. Supervise and provide clinical directions to the HHA/LVN to ensure quality and continuity of service provided. Assure continuity of quality patient care delivered with appropriate documentation
