Develop, review, and update individualized plans of care. Perform routine supervisory visits to monitor caregiver performance and ensure quality, compliant care. Assess clients' health status, identify changes in condition, and recommend appropriate interventions. Collaborate with physicians, caregivers, clients, families, and agency staff to coordinate care. Complete accurate, timely electronic documentation and required reports. Ensure compliance with Maryland home care regulations, agency policies, and professional nursing standards. Qualifications Active Registered Nurse (RN) license in the State of Maryland. Minimum of one year of nursing experience; home health or home care experience preferred. Strong clinical assessment, critical thinking, and communication skills. Excellent organizational and documentation abilities. Reliable transportation and willingness to travel to client homes.
