We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to join our dynamic team. The ideal candidate will provide skilled nursing care to patients in their homes, ensuring the highest standards of care and compliance with all regulatory requirements. Key Responsibilities: Deliver professional nursing care to patients in their homes, following an individualized plan of care. Conduct comprehensive patient assessments and develop care plans in collaboration with physicians and other healthcare team members. Administer medications, treatments, and procedures as prescribed, ensuring accurate documentation and reporting. Educate patients and their families on disease management, medication administration, and preventive health measures. Monitor and record changes in patient conditions, providing timely interventions as needed. Coordinate patient care activities with other healthcare professionals to ensure continuity of care. Maintain accurate and up-to-date patient records, adhering to all regulatory and company standards. Participate in quality assurance and performance improvement activities to enhance patient care outcomes. Provide emotional support and guidance to patients and their families, fostering a positive and supportive care environment. Qualifications: Current Registered Nurse (RN) license in the state of Florida. Minimum of 2 years of nursing experience, with at least 1 year in home health or a related field preferred. Basic Life Support (BLS) certification required; Advanced Cardiopulmonary Life Support (ACLS) certification preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills, with the ability to build rapport with patients and families. Proficient in using electronic health records (EHR) systems. Ability to work independently and manage time effectively. Valid driver's license and reliable transportation.
