COMFORT HOME HEALTH is currently looking to hire an RN, Case Manager for Howard County and surrounding areas. Schedule 15-20 hours per week Up to $30.00 per hour No Weekends Mileage Paid Flexible Schedule NO ON CALL required Education/Skills Graduate of an accredited school of nursing 1 yr. Experience (Homecare preferred) Valid Registered Nursing license with the State of Indiana Communication Skills Time Management Skills Competent with use of PC Responsibilities: Providing skilled professional nursing care to patients per physician's order and in compliance with regulations established by the Indiana State Board of Nursing and Agency policies. Patient Assessment Initiates Plan of Care Document efficiently & effectively Prepares Clinical and Progress Notes Care Coordination Patient Education Supervisory Visits Performs Skilled Nursing tasks as ordered by the Physician on Plan of Care Blood Draws Med Sets Specimen Collection Wound Care Ostomy Care Other: Must have reliable transportation, valid Indiana Driver's License, current Auto Insurance Must pass Drug Screen & National Background Check Job Type: Part-time Flexible schedule Mileage reimbursement Schedule: Day shift Education: Bachelor's (Preferred) Experience: Nursing: 1 year (Preferred) Case management: 1 year (Preferred) License/Certification: RN license in the state of Indiana (Required)
