Home health service is the component of comprehensive health care whereby services are provided to individuals and families in their places of residence for the purpose of promoting, maintaining, or restoring health, or minimizing the effect of illness and disability. PRIMARY PURPOSE OF POSITION: To maintain the client with need of skilled nursing in his/her own home and to maximize his/her functions through the use of nursing interventions while functioning as case manager, directing and coordinating an interdisciplinary team. Providing Nursing Services under the direction of a physician’s plan of care. Serving as the case manager, directing and coordinating the work of a mix of other health care providers. EDUCATION: Graduate of professional school of nursing with state and regional accreditation required; NLN accreditation required; Baccalaureate degree in nursing preferred. EXPERIENCE: One year of recent clinical nursing experience required. Home health nursing experience preferred. LICENSURE AND CERTIFICATION: Current R.N. license in the State of Maine. ANA certification in Home Health or Community Health Nursing encouraged. Current CPR Card (must be Healthcare Provider). OTHER JOB REQUIREMENTS: Must have a reliable automobile. Must have current Maine driver’s license. Independent malpractice insurance strongly advised. Evidence of automobile insurance. Must submit current RN license every two years to Human Resources Dept., on or before expiration date. Must re-train in CPR every two years. Must submit accurate time slips on a weekly basis according to Agency protocol. Attend annual Skills Fair.
