Hospice RN Case Manager

We are looking for a Registered Nurse to care for our patients. Your primary function is to administer skilled nursing care to hospice clients and families in their home. Coordinate care with the interdisciplinary team , patient/family and the referring agency. Perform assessment and assistance with care of the terminal illness. Inform physician and other personnel of changes in client needs and family needs. Assess the ability of the caregiver to manage the client care. Perform on call responsibilities and provide on call services to client and families as assigned. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be compassionate and cool-tempered. You will be able to follow health and safety guidelines faithfully and consistently. The goal is to promote patient’s well-being by providing high quality nursing care. Experience: 2 years (Required) License/Certification: RN (Required) Work Location: Multiple Locations
