Other roles listed: Hospice Case Manager M-F 8-5 SUMMARY: RN is responsible for the management and coordination of care provided to the patients within the in-patient unit, as well as overseeing the general physical functioning of the facility. Acts as the clinical leader in the in-patient unit providing guidance and support for the nursing staff either in leadership or indirect patient care as required with an empathetic attitude for the dying patient and his/her family. DUTIES AND RESPONSIBILITIES: Demonstrates and promotes the highest quality patient care by all members of the patient care team. Provides direct patient care in the HCC as required. Maintains close rapport with the physician concerning patient status. Monitors and promotes routine visits during the patient’s hospitalization. Ensures that physicians see acute care patients in the HCC every 48 hours and that physicians’ orders are signed off within 48 hours of admission. Responsible for all medications and treatments provided by team members. Regularly inventories and keeps adequate medication stock available. Responsible for patient and staff safety and is always alert to environmental and mechanical problems. Reports these to the Director and/or maintenance staff. Participates in coordinating fire drills in the facility with particular attention to the HCC as assigned by the Director. Encourages the use of good body mechanics while providing patient care.
