Registered Nurse (RN) Job Description

Must be able to: 1) Provide a regular written schedule of availability at least weekly but preferably monthly. 2) Follow a Holiday on call coverage per rotation schedule. Responsibilities: Perform an initial evaluation of the patient in regards to the impact of the terminal diagnosis utilizing the nursing process. Determine the frequency and duration of services based on the patients physical, functional, psycho socia, and environmental needs as well as activities of daily living. Assess the need for interdisciplinary referrals; initiate and document the referral. Implement and document treatments per physician’s orders. Initiate the appropriate preventative and rehabilitative nursing procedures. Educate the patient and family on disease process, end of life care and the process for dealing with ethical issues. Provides liaison with nursing services of hospitals, skilled nursing facilities and the medical community. Consult with and assess the family for risk of pathological grief, cultural and spiritual issues, and educational needs. Assess the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout the care. Provide on-going assessment of the patients/family response to care. Inform the physician and other members of the interdisciplinary team of changes in patient needs and outcomes of interventions. Update the plan of care to reflect changes in care. Develop an aide plan of care with specific interventions and goals. Apply specific criteria for admission and recertification to hospice care to establish appropriate levels of care and the patient’s eligibility. Prepares clinical and progress notes that demonstrate progress toward establish goals in the plan of care. Perform chart reviews as required. When designated, meet with the IDT for record review and attend team meetings as required by Supervisor. Attends all mandatory education and in-service programs. Supports the organization by adhering to company and regulatory policies and procedures. Participate in the on-call system. Qualifications: Associates Degree in Nursing from an accredited school of nursing preferred Must possess a current, valid RN license in state of practice Certification in CPR/ BLS 2 years of experience in a hospice preferred Excellent communication skills Demonstrated ability to maintain a high level of professionalism during stressful times Strong critical thinking skills Dedicated to provide the best patient care possible Our Benefits include: Medical, Dental, Vision, STD and Life Insurance (FT St
