Registered Nurse (RN) Job Description

Under the direct supervision of the D.O.N., the Charge Nurse is responsible for all direct patient care in a 60-bed facility. Assess each individual’s condition as you make rounds, be alert to any changes that resident’s condition and response to medications and treatments. When accidents occur, record description of bruises as to size and color, etc. Record size of area of skin tears or lacerations. React to emergencies promptly and request necessary help, etc. Supervise in dining room during meals, administer medications and note reactions. Accompany physicians on rounds to answer any questions, receive instructions and note resident care requirements. And take care of orders received. Answer call bells as necessary. Assist with patient care as necessary. Supervise staff to provide good care and keep the environment safe and clean. Qualifications: 1. Graduate from an accredited school of vocational nursing 2. Current license from the State Board of Vocational Nurse Examiners for the State of Texas. 3. Three years of experience in general acute nursing, preferably with the older adult. 4. Two years of experience in a long-term nursing care facility. 5. Must be CPR certified. 6. Must be able to follow and understand oral and/or written English directions. 7. Must have computer knowledge. MAJOR RESPONSIBILITIES FOR ALL SHIFTS: Knowledgeable of all nursing/administration policies and procedures. Obtain report and read log. Make resident oriented rounds, assess each individual as you do so. Transcribe all new orders. Administer medications as ordered and record accurately and note reactions. Administer P.R.N. medications and record response. Account for all medications and controlled substances. Call physicians as necessary. Receive calls from physicians. Receive calls from families, answers inquiries courteously and promptly. Answer call lights as necessary. Check every eight hours for accuracy of medication sheets. Complete accident/variance report as needed and notify physician and responsible party of incident. Follow up on accident and variance report as indicated. Manage all transfers as they occur. Record all treatments administered, including feedings. Change indwelling, catheters as required. Complete Nurse’s Notes as indicated for each classification. Giver report to on-coming shift. Keep medication cart, medication room and desk clean and orderly. Maintain cleanliness and order of unit and resident rooms. Observe and record nutritional status of residents. Respond to emergency calls from the residential and apartment areas. Provide medication supervision to residents in the residential and apartment areas as indicated by physician. Supervise in dining room and during meals. Admit new residents. Notify physicians of any lab reports Supervise the weighing of each resident monthly or as ordered by the physician. Order and/or stock medications and supplies as required. Complete 24 hour infection control surveillance report and forward to D.O.N. Complete weekly summary on all residents. 60-bed facility All employees will be required to receive or show proof of receipt of an approved COVID-19 vaccine as a condition of employment, except where a legally authorized religious or medical (not personal preference) or otherwise legally protected exemption is established.
