To conduct nursing assessments, monitor health problems of center participants, and create nursing care plans;
To provide health education and counseling to program participants and their families, conduct health related training to ADHC staff and volunteers, and serve as nursing consultant;
To refer program participants to appropriate healthcare providers in the community and serve as a liaison with such organizations;
To serve as a key member of the Multidisciplinary Team that provides: Professional Nursing, Social Services, Therapeutic Activities, Physical Therapy, Occupational Therapy, Speech Therapy, Mental Health, and Dietary services to program participants.
