Registered Nurse (RN) Job Description
The successful Home Health RN has a passion for working 1:1 with patients, in their home environments, to maintain or improve their health status. Physician's orders are the basis from which home health plans of care are designed. Case coordination brings all disciplines together with targeted focus on the patient. A home health RN can achieve a life-work balance due to personalized scheduling with the clients assigned. The hours are Monday - Friday, 8:00 - 4:30, with a light on-call rotation with co-workers. QUALIFICATIONS • Current licensure as a Registered Nurse required • At least two years of experience in an acute care, long term care or community health nursing setting. • CPR/BLS certified • Valid driver’s license and automobile insurance • Mandatory child/adult abuse reporter status current The essential job functions include: I. Manages a caseload of assigned patients receiving home care services in accordance with accepted policies and standards. • Performs comprehensive assessments, identifies patient/family needs and implements plans of care • Coordinates various providers and community resources to help meet patient/family needs. • Communicates with physicians and other care providers on an on-going basis; manage and implement orders given by the medical community • Demonstrates respect for cultural differences and diversity II. Provides skilled nursing, educational and other services to individual patients and families in the home or other settings. • Performs on-going physical, psychosocial, and other assessments/evaluations, initially and on an on-going basis as required for the EHR • Develops and implements a plan of care for each patient with identified goals and interventions to meet patient needs in accordance with standards and guidelines. • Performs ordered venipunctures, infusion therapies, wound care, and other procedures according to accepted standards. • Consistently provides patient, family, caregiver education and instruction as needed and at a level appropriate to their needs. • Manages all information in the EHR accurately and timely
