Apply today and become a valuable member of Modivcare PCS. Care Finders Total Care LLC, a Modivcare Home care company in NJ is looking for a Per Diem Clinical supervisor. The RN field nursing supervisor is responsible to ensure quality and safe delivery of home health Care services such as supervising Certified Home Health Aides in a patient’s home, evaluate the patient’s plan of care and assess their physical and Psychosocial needs.
Schedule: 1–2 days per week, daytime hours, covering Morris County and Bergen County. Visits will be scheduled with the patient’s aide present as required.
Office Location: 466 Bloomfield Ave., Suite 200 Newark, NJ 07107
Compensation:
Regular Visit, 2-Month Reassessment, and Supervision Visit: $45 per visit
Reassessment Visit: $55 per visit
Initial RN Assessment: $60 per visit
Mileage Reimbursement: Provided at the current federal mileage rate.

You will...
- Performs on-site supervisory and teaching visits to evaluate caregivers
- Provides clinical orientation to caregivers on assigned home care cases.
- Participates in the training of new nurses and in-services for field staff.
- Performs initial nursing assessments
- Counsels patient/caregiver in meeting nursing and related needs.
- Assigns and coordinates services provided.
- Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.
- Prepares clinical progress notes ensuring any changes in the patient’s condition is reported
- Administers medication and treatments as prescribed and initiates appropriate preventative and rehabilitative nursing procedures.
- Monitors clinical records for complete and accurate documentation.
