Registered Nurse (RN) Job Description

Essential Functions: • Utilizing the nursing process makes the initial evaluation visit to assess patient needs following the conditions of participation guidelines. • Develops and initiates the plan of treatment, carrying out the physicians’ instructions. • Regularly re-evaluates the patient’s nursing needs making necessary updates and revisions and report patient’s condition to appropriate member of the health care team. • Performs general and specialized therapy (including infusion, wound etc.) procedures as appropriate. • Initiates appropriate preventative and rehabilitative nursing procedures. • Prepares clinical and progress notes using an electronic medical record. • Coordinates and supervises Home Care Aides and documents weekly review of progress notes. Attends Case Conferences. • Coordinates Physical, Occupational and Speech Therapy reviews therapy documentation and care provided. Holds case conferences as needed. • Counsels and instructs the patient and family in meeting nursing and related needs and goals. • Participates in in-service programs and teaches home care aid and family members care techniques.
