RN Field Nurse Home Care

Summary: New York State Licensed Home Care Services Agency (LHCSA) is seeking a full-time RN Field Nurse. We offer a competitive salary, health benefits, and a 401K matching plan. Key Responsibilities - Perform client assessments, review physician orders, and create care plans - Prepare, record, and maintain clinical and progress notes in the client care record - Serve as case manager, responsible for coordinating care for assigned caseloads - Observe signs or symptoms indicative of change in condition to the physician and document such reports and observations in the client record and update the care plan - Educate clients and family members on resources, medications, self-care, and/or medical conditions per physician orders - Identify discharge-planning needs as a part of the client care plan and implement according to physician orders - Play a key role in clients avoiding unnecessary visits to the ER and or hospitalizations - Perform case supervisory visits on a periodic basis throughout the year - Act as a role model for new caregivers, accompanying them on home visits and providing mentorship - Meet required deadlines for documentation of certification, re-certification, supervision reports, care plan updates, addressing progress towards goals, and physician orders as needed - Build personal rapport and establish trust with clients, family members, and caregivers Requirements: - Registered nurse with current licensure to practice professional nursing in New York State - Must possess and maintain current CPR certification - Knowledge, ability, and judgment to perform all nursing procedures under the direction of a physician as outlined in the scope of practice in the New York State Nurse Practice Act - Meet the requirements for employees providing direct client care - Valid driver’s license and use of an insured automobile for work or access to adequate transportation Compensation and Benefits: - Full-time Salary: $100,000 per year - 401 (k) retirement savings plan, with employer matching - Health insurance - Flexible schedule - Opportunities for growth
