This is a wonderful facility. I actually was very worried when I had to leave my husband after he had a stay in the hospital. All of the staff took great care with my husband. The therapists went out of their way to find good timing for my husband when he was focused enough to participate. My husband got to recognize all of the staff working with him. They always got smiles from my husband and always gave smiles to my husband. Overall a very kind and caring staff. I appreciate all their hard work.
My brother was at South Creek for 2 months. Initially the care was marginal but care change to neglect facilitated by lack of staff. When asked to correct simple tasks for activities of daily living such as assist out of bed, help with meals, water and consistent therapy the comment was "we are short staffed". The neglect lead to a life threatening hospitalization which could have been avoided with care and supervision . I would avoid this SNF.
This place deserves no stars honestly if I could say zero I would say zero so within the last 24 hours I finally found out why everybody else in the community is not a fan of this place we had to pull my father out of this place just this morning 2 to the fact of him falling down twice while in the care of this place.seems crazy right, sure made my heart sink, To see my dad in pain and threatening to call the police come on now do you know better than to do that if you can't even pay attention to your own personnel. Worst facility if you have other choices please go with them and not this Care facility
Family member was put there from hospital without family knowledge. We were told they were given family info but received no call he was there. When admitted he was walking and after 41 days of what was supposed to be rehab turned out to be he was wheelchair and bedbound. They did not get him up and get him moving like they were supposed too. Even though he had a power of attorney, they refused to call the power of attorney and give updates after they said they would. They did not notify the family that he was going to be charged for his stay until he owed several thousand dollars. Maybe if they would have been in contact with his POA and told them after so many days in he will charged we could have prepared for him to come home. Calling that place is a joke as they never answer the phone. When he made the choice to leave because he couldn’t afford it, they called the sheriffs department on us stating that he was in danger of his family for him making his own choice to leave. Sheriff came to the house and made several attempts to break in even though it was obvious no one was at home. The facility called 911 before we were even off their property so it did not allow us to get home before the sheriff was there. We have tried to retrieve the clothes that were in the laundry when he left twice and no one will call us back. After 30 days they say they donate or discard the belongings. We have requested all of his chart notes and the pictures of his wounds so we could turn them into his doctor and he wants a copy for himself and we keep being told they will and they don’t. The first excuse was they didn’t have the emails to send info too which they had and then it was they needed the POA paperwork which they had had all along. I would not recommend this place for my loved ones. How do you say someone’s in their right mind and then turn around when it doesn’t profit you and say he cannot make his own decision to leave? This is not my first time having a loved one at a skilled nursing facility. Over the last two years this is the fourth one I have had a family member in and I will tell you it is one of the worst. Please, if you care about and love your family, do not put them there.
If I could give negative stars I would. I NEVER leave reviews but I couldn’t let this go without warning others. We came to this facility after my father's hospital stay, thinking he could benefit from some strength-building before returning home. What we found instead was deeply disturbing. The moment we walked in, the facility reeked of neglect. Hallways were crowded, with multiple patients seemingly hanging off their beds, calling out for help with no response from staff. One lady looked at me and asked for water. At the front desk, we were met with cold stares and told the room number with no escort, no welcome, no assistance. When we arrived at the room, it was worse. The TV was broken, the room was shared with another patient who informed us that he hadn't had his sheets changed in three days, only after his wife demanded it after she noticed the same stain for those three days. The roommate informed us four patients shared one bathroom, and the room was hot which made it incredibly difficult to sleep. The nurse came in after ten minutes, did nothing, and disappeared again for another twenty. The patient next door had their TV blaring, and we were told this happens constantly, making it impossible to sleep. The roommate’s wife told us the staff was inattentive, likely due to being dangerously short-staffed. This was very concerning for my family considering my dad needs assistance to eat, use the restroom, stand, etc. I did not feel my father would be well looked after here. After seeing and hearing all of this, we looked up more reviews online and were horrified. We made the clear and united decision as a family that there was no way we could leave my father here. We have set up his care at home, including a custom chair, grab-bars, and other modifications to support his recovery. My mother, a nurse with over 30 years of experience including rehab and nursing facilities, knew instantly that this environment would do far more harm than good — mentally and physically. When we told the nurse of our decision, she brought in three staff members to what can only be described as an interrogation. One of the admin staff members was downright rude and condescending to my mother, who was simply trying to explain our reasons for leaving. The other two simply stared at us from the doorway, keep in mind we hadn’t even seen nor met these people. They then insisted that my father sign documents even though he could barely hold a pen — just because we had “entered the building” and were “planned to be admitted,” despite the fact that no formal admission had occurred. To make matters worse, after we left and got my father safely home, we were shocked to receive a call from the sheriff’s department for a welfare check. Apparently, someone from the facility reported us, despite the fact that we explained everything clearly and had full legal and medical justification for taking him home. It was outrageous and added unnecessary stress and trauma to an already difficult day. The only person who showed any compassion was a single nurse who was kind to my father. But beyond that, this place left us stunned, angry, and heartbroken. If this is how they treat patients and families before someone is even admitted, I fear for those who are already stuck inside. We will never step foot in this place again. If you love your family, I urge you to think long and hard before considering this facility. What we experienced was unacceptable — and frankly, shameful.
