Part of Phoenix Health Services, a family owned and operated healthcare company, Phoenix of Maple Heights is tucked away on a quiet cul-de-sac in a wooded residential neighborhood of Cleveland, Ohio. We strive to create a community that is inviting and engaging to each patient who walks through our doors. Our community is within five miles of three major hospitals and easy to reach via I-271 and I-480, making visits from family and friends an enjoyable trip.

I have visited this facility for over 3.5 years, so I’ve had the opportunity to observe daily operations firsthand. I'm always impressed by the level of care provided! The environment is always clean, well-maintained, and welcoming -- which makes a significant difference for both residents and staff. The activities dept does a great job keeping residents engaged, active, and happy, and the result is a community where residents form real & genuine relationships with one another. The nursing staff's consistency, compassion, and clinical skills are evident in everything they do. You can tell they truly care! The food is consistently tasteful and presentable. The dining rooms are always full during mealtimes as residents enjoy eating together in a social, comfortable setting. The kitchen staff always goes above and beyond to ensure each residents is served a tasteful, filling, and balanced meal. Management is always accessible and responsive. Michelle, the administrator, truly cares for each and every resident... she forms relationships with every resident and their family members. I've witnessed firsthand that the leadership team collaborates effectively to support the wellbeing of every resident. Their teamwork is evident! Overall, this facility demonstrates a level of care and professionalism that truly stands out. It’s always a pleasure to visit such a dedicated team!

Deja, you seem very angry, but your portrayal of Phoenix of Maple Hts. is simply not truthful. Michelle the administrator is always available and gives everyone her cell phone number. It's evident you never attempted to call her because she would ensure she addressed every concern you expressed. It's very easy to lash out when your angry but its not right to lie. I had my aunt there after she was hit by a car. Her care and therapy was good and she was able to return to independent living. I am not saying they are perfect but I know when my aunt wanted something particular from the kitchen, the manager came to speak to her and accommodated her. As far as them not notifying you, unless your loved one has a guardian who they wound notify first, I was always kept in the loop with any changes. The DON also was very responsive. I've been a nurse a long time and I was very pleased with my aunts stay. Staff was friendly and gave better care then she had at the hospital. I'm grateful to the staff at Phoenix of Maple Hts and would highly recommend them.

Effective leadership is the heartbeat of any successful facility, and at Phoenix of Maple Heights, that heartbeat thrives. The exemplary leaders here, Katrina, the Director of Nursing, and “D,” the Assistant Director of Nursing, are always just a question away, ready to address any concerns with genuine care. Their pride in their roles sets the tone for the entire team. The nursing staff goes far beyond merely administering medication; they forge strong connections with the aides, working hand in hand to provide residents with thoughtful, comprehensive personal care and compassion. My brother was there for rehab after leaving a hospital where he was recovering after falling and injuring himself. He has been in other rehab facilities in fancier surroundings where he constantly said he wanted to leave. When he was at Phoenix (the only rehab place he never complained about), he said the staff was great and paid attention to his needs. He especially appreciated how nice the Administrator (Michelle) was to him. Michelle also interacted regularly with me to keep me updated on my brother's progress and did her best to make sure he was well taken care of. This review is not meant to denigrate other facilities. Rather it's meant to praise the good care my brother received at Phoenix in Maple Heights.

The ownership of the company are the administrators and have a good pulse on the daily care. Phoenix of Maple Heights is clean! I recommend them to anyone needing to place a loved one.
