My mom is in this facility and I haven’t heard from her in months after making a complaint about her care. I live all the way in Louisiana and my mom has no family up there. She used to tell me when I talked to her that they were mean to her. She was sent to the hospital for a kidney infection and when she was released back to the nursing home her health didn’t get better. She got septic and went back to the hospital. Dr said that if her antibiotics would’ve been given to her she would have gotten better. I had to make a trip up there just to check on her well being!! I made a complaint and now “she’s been moved to another wing” of the hospital and “getting better care” but I haven’t heard from her in months and when I call they put me on hold for so long their phone system goes to voicemail or it hangs up on me. I’ve sat on hold for 20-30 minutes multiple times for this to happen. My mom’s left side is completely paralyzed from multiple strokes.

I'm giving one star because overall it should be ZERO but several of the nurses seemed to be well trained and caring, however the administration is horrendous. My elderly mom with Alzheimer's ended up there after she was in the hospital for a heart attack. The problem started by her testing positive for COVID when admitted to the ER, which required she be placed in the COVID ICU. (By the way, my mom had been double vaccinated but we don't know if everyone at her facility is vaccinated.) After a few days in the hospital, I insisted the they test her again. She tested negative and remained in the hospital for about six more days. However, because she originally tested positive, no rehab facility would take her with the original COVID diagnosis and Viviant was the only facility with a COVID ward. Granted, the hospital COVID experience is horrible for a person with Alzheimer's having to be completely isolated. However, after arriving at Viviant she dramatically went downhill. As others have said, Viviant rarely answers the phone. On the few times, someone answered the phone, our call would need to be transferred the single phone in the COVID ward. Those nurses are busy (very busy!) yet they are the only link to try to find out how your loved one is doing. After about a week, we finally got a call from a social worker at Viviant who said she had no contact information for my mom. Really??? An ambulance just dropped her off?? Someone sent my sister paperwork to fill out the day mom was submitted, but this social worker evidently had no information. Mom was in a room with another COVID person, which surprised me that is protocol, and Viviant had no idea mom tested negative over a week prior. They also didn't know her medical history or the heart attack that sent her there. The social worker said mom would be moved into a non-COVID room after 14 days and refused to test her because she said tests are only conducted every 90 days. It all sounded totally crazy to me. Here's some of the information we got about my mom's condition...one of the nurses said mom had typical COVID symptoms (I had to correct and inform her), another said mom was doing great and walked to the shower herself (mom hasn't done that in years), another said she was going downhill fast, which is when we knew we had to get her out of there. It was like a Kafka novel because no other facility would take her and her home facility wasn't prepared for her in an unknown condition. We decided it was time to put mom on hospice and I got the county ombudsman involved to expedite the transfer back to her home. When I initiated the transfer and the doctor associated with the facility (who probably never even saw mom) signed off quickly but the social worker said there were no ambulances available. I got the ombudsman involved again and all of a sudden my mom had an ambulance. Mom was so happy to be back home and did ok for about a week. She rallied and tried hard to recover but after a month in COVID wards with very human interaction was too damaging for her. She passed last week. Viviant needs oversight. We had a similar situation with my dad's facility 7 years ago and I made several complaints to the TN Dept. of Health, which went nowhere. I'm not bothering with formal complaints against Viviant because I know the State doesn't care. Google reviews matter. Be careful placing your loved one here!

I moved my family members here and I have been very pleased with all the staff, the nurses, food service, maintenance, social worker, therapy, nursing management and nursing staff are very pleasant, and responsive to the needs of the residents and family. The food is really good most of the time. I wish the bathrooms were larger and ADA accessible. My mom is able to walk with a walker after being unable to walk for over 5 yrs. I'm glad I found Boulevard Terrace.

Viviant has been so Kind and Nurturing to my Mother. She is from Robertson County and has become a part of their family. My Mother wants to stay at Viviant. They LOVE her and she LOVES them! The Administrator, Armand Santos, has been so very helpful and a GREAT liaison in her care…

The facility has made a 360 since the new owners have taken over and the building is undergoing renovations! The progress has been wonderful and they have a great team of specialist that come to facility to give specialty care from podiatry, psychiatry, advanced wound care providers, and a house provider and NP. The leadership within the building is strong and patient focused to ensure optimal outcomes!
