Nhc Place at the Trace – Medicare Certified Since 2016

We strive to make every moment count for our residents and patients at NHC Place at the Trace because we believe in celebrating life at all stages. Short Term or Home. Our residents and patients come to us for a variety of reasons. Many are short-term, having left a hospital needing skilled nursing and/or rehabilitative care to get them back home. Other residents have chosen us as their home with assisted living care, long term care, or memory care. The NHC Place at the Trace’s campus is home to a 90-bed HealthCare Center, a 80-apartment Assisted Living community, and our 20-apartment WayPoint Memory Care. Our campus has a restaurant-style dining rooms, landscaped courtyards and recreational amenities. Our Promise. We are dedicated to offering our residents and patients the independence they want with the support they need. In fact, all NHC partners prepare for their workday by reciting one of our 20 promises each morning. Our vision is Caring in a Better Way day by day.

I can’t speak highly enough about NHC Place at the Trace! I spent two weeks there for physical therapy after I broke my hip. The nurses, care partners, physical therapists and occupational therapists are excellent! I was met with kindness, compassion and respect from everyone involved- without exception! I highly recommend them!!

Exceptional Care and Compassionate Staff I can’t say enough wonderful things about NHC Place at The Trace. From the moment you walk through the doors, you’ll be welcomed with warmth, professionalism, and genuine care. The entire team—from the front desk to the social work team, nursing staff, therapists, and administrator —goes above and beyond to make residents and families feel supported. It’s rare to find a facility that delivers both top-tier clinical care and a deep sense of community, but The Trace does just that. I wholeheartedly recommend them to anyone seeking compassionate, high-quality care for their loved ones.

This is the second time my mom has been in this facility for rehab. The staff is great, but one person stands out to mom and I. Ka Ka is a young lady who often goes above and beyond what you expect in a very caring compassionate way. Ka Ka remembered mom and gave her a hug. This is someone who puts the family at ease when they can’t be there.

We've had a couple family members at this location and have been overall very satisfied. Staff is friendly and respond accordingly. The food is good.

Negligent to check on my mother throughout the night, resulting in me not being able to sleep for four years. If your loved one cannot access the call button and is not able to get around on their own, move them to another facility or have someone sit with them. Staff is slow to check in on patients.
